Incident Investigation Request Form

The deadline has passed to submit an incident investigation request if you tested on or before the 26th September. Requests submitted more than 5 days after your test date will not be accepted.


Please read the Test Incident information before submitting your request. There may be a delay in dealing with your request if you do not provide all the information required.

Complete the form below to request an incident investigation. You will need:

  • Your candidate ID (in the format UKCAT + 6 digits)
  • Your incident case number (given to you at the test centre)

Do not exceed the character limit (2,000) when giving details of the incident or the form will not submit.

Your incident investigation request has been submitted directly to Pearson VUE Customer Services.

Once a request has been logged, your case will be investigated by Pearson VUE.

If you have not received an outcome within 5 working days (and it is not in your spam/junk folders), please email Pearson VUE Customer Services and quote your candidate ID or case number.
